Two ways to use this sheet

Keep one card for one book

Use an upper card for the title, date and a brief reaction. Use a lower card for a second thought, a reading memory or your own question.

Build a small reading archive

Keep the cards loose or arrange one with the RA02 pocket. Compare the card with the folded opening before gluing; physical insertion is still awaiting verification.

Choose the prompts you need

All four cards are 78 × 108 mm, but they do not all have the same prompts. Cut the outer rectangle at Actual size. Choose RA03 if you want room for a longer response.

Choosing and using this printable

Are these four different book review forms?

No. Two cards share the book review prompts and two share a general note layout. The illustrations differ, but the writing functions repeat.

Is this a multi-book reading log?

No. Each prompted card is for one book. There is no printed table for tracking several books, ratings or reading totals.

File notes

AI-assisted illustrations; text, linework and millimetre layout are controlled separately. The previews are rendered from the PDF.

Revision r0.9: Component IDs moved into trim waste. Artwork, writable areas and cut/fold/glue geometry preserved; physical checks pending. Physical evidence is still pending for this revision.
